Provide Visit Guidelines

Provider fees may include:

  • Office visits: approximately $50
  • Urgent care visits: approximately $75
  • Specialist visits: approximately $75
  • Outpatient services: approximately $75
  • Emergency room and inpatient services: approximately $150

Sharing Structure

Household Primary Responsibility Amounts (PRA) may be available starting around $2,500 per year.

After the PRA is met, eligible medical expenses may be shared among members, often through a co-share structure.
